Each child needs their own appointment. Please do not include more than one child per submission. Return to this page to schedule an additional child for the same day.
If your child has had a fever or other illness within 24 hours of your appointment, please call the office to reschedule your vaccination.
Also note, vaccination appointments are scheduled with a nurse. If you have other concerns that require seeing the doctor, please call the office to schedule a sick visit.
When you arrive for your vaccination appointment, please go directly to the main office waiting room.
